Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
- Medicals for pre-employment and driving requirements (HGV, PSV etc.)
- Insurance claim forms.
- Prescriptions for taking medication abroad.
- Private sick notes.
- Vaccination certificates.
- Reports.
- Holiday cancellation certificates.
- Fitness for sports activities.
- Access to records.
These fees are based on the British Medical Association (BMA) suggested scales, for more information please ask reception.
Fees List
Certificates
- Certificates, forms, letters, applications requiring dictation and / or a check of patient record – £40
E.g. Fitness for Travel, Private Insurance Certificate, Travel Cancellation, University Occupational health form, vaccinations requiring a GP signature.
- Witness of signature face to face in an appointment – £50
- Firearms license forms – £100
(If any of the above also require a 10 minute appointment with the form, an additional £10 will be charged)
Examinations
- Pilot, HGV/PSV, Pre-employment, Minibus, Taxi - £150
Reports
- Detailed Reports – £150
- Basic Report – £100
- Ofsted Health Declaration/Childminder – £125
- Insurance medicals & dictated insurance/employment reports – £150
(If any of the above also require a home visit, an additional charge will be added)
We do NOT provide letters or medicals for parachuting, golf buggies, diving or pregnancy fit to fly.
