Our new long term condition appointment system is designed to review all your long term conditions in one appointment.

This will be a physical health check with one of our Healthcare Assistants. This may include a blood test. If you have a blood pressure monitor, please bring a week’s worth of readings to your appointment. If you are diabetic, please bring a urine sample (Sample bottles can be collected from reception)

Following this appointment, you will have a pharmacist medication review.

If you have Diabetes or COPD you will also be booked in with one of our specialist nurses.

Please note that you will only receive a message or telephone call from the diabetes/COPD nurse or pharmacist and will not attend the surgery unless advised otherwise.

Please tell us when you would be able to attend the surgery for an appointment within the next 4 weeks.
