Blood Pressure Readings

Click “Submit my readings” to submit your blood pressure readings. You will be taken to another page where you will need to click “Existing Condition” then click “Submit Blood Pressure Readings”

Remember: Record your blood pressure for 5-7 days in a row. On each day, record your blood pressure in the morning and in the evening. On each occasion take at least 2 readings, leaving at least a minute between each, recording the systolic, diastolic and pulse .
